Home from Hospital
The aim of this new service is to provide short-term support to older people who have been discharged from hospital, suffered a recent illness or who have had a fall.
The focus is on rehabilitation with the client being encouraged to resume activities of daily life in a phased way as health and for confidence improves.
Referrals to the service will come from the Intermediate Care Assessment Team or other heath and social care professionals.
The service will provide a range of support to meet clients’ needs, such as:
- Settling in and greeting Home
- Assistance with shopping.
- Moving bedroom downstairs
- Ensuring mobility equipment is in place
- Dealing with correspondence and debt
- Re-instatement of benefits
- Emotional support and companionship
- Assistance with meal planning and cooking
- Assistance on short trips
- Assistance with housework
- Arranging of appointments
- Referrals and links to other services
Clients will have a dedicated support worker to agree with the client a care package. The service will provide up to 2 hours per day from Monday to Friday up to a maximum of 6 weeks.
The service does not provide
- Personal care
- Administer medication
- Taking home from hospital
- Large scale cleaning tasks
- Removals
